Associates, we value you as a patient and hope to make your visit in our
office as pleasant as possible.
No Shows/Appointment
Cancellations
If you are unable to keep
an appointment, we ask that you provide us at least 48 hours notice.
This will allow us to give your appointment time to another patient.
Failure to notify the office may result in a $25 service fee.
Scheduled Surgical
Procedures
All patients will receive an initial consultation prior to being
scheduled for surgery. If you are unable to keep your scheduled surgical
appointment you must notify our office at least (5) business days prior
to your scheduled surgery. Failure to notify the office will result in a
$250.00 service fee
Co-Pays and Past
Balances
Co-pays are DUE AND PAYABLE AT THE TIME SERVICES ARE RENDERED. Please do
not ask to be billed. All patients are required by their insurance
carrier to have their co-payment and/or co-insurance information at the
time of services (please refer to your handbook provided by your
insurance carrier). Tri-County Surgical Associates will be in violation
of our contract with your carrier if we do not collect your co-pay
and/or co-insurance at the time of service. We will make every effort to
work with you regarding any past due balances.
Insurance
Policies/Referrals
We participate with most insurance plans; however, it is your
responsibility to contact your insurance carrier to determine whether or
not we are a participant in your plan. Insurance companies classify
Tri-County Surgical Associates as a specialty practice, and many
insurance carriers will not pay for specialty services without a
referral from your primary care provider. If your plan requires a
referral from your primary care provider for specialty office visits,
YOU are responsible for obtaining this referral prior to your scheduled
appointment. Managed Care plans require patients who do not have their
referral at the time of appointment to either reschedule their
appointment or make payment at the time of service. Medicaid patients
who have exhausted their allowed number of visits will be responsible
for their visit or may be rescheduled when additional days are granted.
Worker’s Compensation/Forms (Insurance/Long Term Disability)
When making an appointment involving workman’s compensation, verifiable
contact information must be provided to our office when scheduling your
appointment. We will make every effort to assist you in obtaining the
authorization for your office visits. If we are unable to obtain
authorization, other payment options are available. There is a $20.00
fee for completing all Insurance/Long Term Disability forms. This fee
must be paid at the time the forms are submitted. All forms must be
turned into the receptionist, we cannot accept any forms which are faxed
or dropped into the mail slot. If you wish to have your form faxed to
you after completion, please provide a valid fax/phone number upon
submission.
Appointment Delays
Due to the nature of our practice, the Physicians may be delayed because
of unforeseen events or emergencies. You will be notified upon check in
if the Physician you are scheduled to see is delayed. If your schedule
does not allow you to wait, you will be given an option to reschedule at
the next available appointment time. We will make every effort to keep
you informed; please notify the receptionist if you are not seen within
30 minutes of your check in time.
Prescription Refills
When requesting a refill
of your medication please allow up to 24 hours for refills to be called
in. We do not call in refills after 2:30 P.M. on Fridays or on the
weekends.
